Dental Implant Placement Post-Op Instructions

Following these instructions carefully after your dental implant placement will help ensure proper healing, reduce discomfort, and give your implant the best chance of long-term success. If you received IV sedation during your procedure, you must have a responsible adult drive you home and stay with you for the remainder of the day.

Immediately After Surgery

Bite gently on the gauze pad placed over the surgical site for 30 to 45 minutes. If bleeding continues, replace with fresh gauze and maintain gentle pressure for another 30 minutes. Some oozing and blood-tinged saliva is normal for the first 24 hours. Avoid spitting, using a straw, or rinsing vigorously during this time, as these actions can dislodge the blood clot forming at the surgical site.

Managing Swelling

Swelling is a normal part of the healing process and typically peaks 48 to 72 hours after surgery. To minimize swelling:

  • Apply an ice pack to the outside of your face over the surgical area (20 minutes on, 20 minutes off) for the first 24 hours
  • Keep your head elevated when resting or sleeping for the first two to three nights
  • After 48 hours, switch to moist heat (a warm washcloth) to help resolve remaining swelling

Pain Management

Take prescribed pain medication as directed before the local anesthesia wears off. If Dr. Brumbaugh prescribed antibiotics, take the full course exactly as directed even if you feel fine. For mild discomfort, over-the-counter ibuprofen (Advil or Motrin) taken as directed on the label is often sufficient. Do not take aspirin, as it can promote bleeding.

Diet and Nutrition

Proper nutrition supports healing. Follow these guidelines:

  • First 24 hours: Cool, soft foods only: yogurt, smoothies (no straw), applesauce, mashed potatoes, protein shakes
  • Days 2–7: Soft foods that require minimal chewing: scrambled eggs, pasta, cooked vegetables, fish, soup (not hot)
  • Weeks 2–4: Gradually reintroduce firmer foods as comfort allows, avoiding the implant site when chewing
  • Avoid throughout healing: Hard, crunchy, sticky, or spicy foods near the surgical area; extremely hot foods and beverages

Stay well hydrated with water. Avoid carbonated beverages for the first few days.

Oral Hygiene

Keeping the surgical site clean is critical to preventing infection and ensuring successful integration of the implant with your jawbone:

  • Day of surgery: Do not brush near the surgical site. You may gently brush other teeth.
  • Starting the next day: Rinse gently with warm salt water (½ teaspoon of salt in 8 ounces of warm water) after meals and before bed. Let the rinse flow out of your mouth rather than swishing forcefully.
  • If prescribed: Use the chlorhexidine (Peridex) rinse as directed, typically twice daily for two weeks.
  • After one week: You may carefully brush near the implant site with a soft-bristle toothbrush, being gentle around the healing tissue.

Activity Restrictions

Rest is important for healing, especially in the first 48 to 72 hours:

  • Avoid strenuous exercise, heavy lifting, and bending over for 3 to 5 days after surgery
  • Do not smoke or use tobacco products. Smoking significantly increases the risk of implant failure and delayed healing
  • Avoid alcohol for at least 48 hours and while taking prescription pain medication
  • Do not wear removable dentures or partials over the surgical site unless Dr. Brumbaugh specifically advises otherwise

What to Expect During Healing

The following are normal after implant placement and should not cause alarm:

  • Mild to moderate swelling for 3 to 5 days
  • Bruising on the skin near the surgical area, which may take 7 to 10 days to resolve
  • Slight bleeding or oozing for the first 24 hours
  • Numbness or tingling near the site that resolves within a few hours as anesthesia wears off
  • Stiffness or limited jaw opening for a few days
  • A slight increase in body temperature the first evening

Signs That Require Immediate Attention

Contact our office at 214-369-5159 right away if you experience any of the following:

  • Bleeding that does not stop after sustained pressure with gauze
  • Pain that worsens significantly after the third day
  • Fever above 101°F that persists beyond the first evening
  • Pus or foul-smelling discharge from the surgical site
  • Numbness that persists beyond 24 hours after surgery
  • The implant feels loose or the healing cap comes off
  • Difficulty breathing or swallowing

Long-Term Implant Success

After the initial healing period (typically 3 to 6 months), your implant will integrate fully with your jawbone through a process called osseointegration. During this time, attend all scheduled follow-up appointments so Dr. Brumbaugh can monitor healing with periodic imaging. Once your final restoration (crown, bridge, or denture) is placed, maintaining your implant requires the same habits that protect natural teeth:

  • Brush twice daily and floss around the implant with an interdental brush or water flosser
  • Attend regular professional cleanings every six months
  • Avoid using your teeth as tools or chewing ice
  • Wear a night guard if you grind your teeth

Follow-Up Appointments

Dr. Brumbaugh will schedule your first post-operative visit approximately 7 to 14 days after surgery to assess healing, remove any sutures if necessary, and ensure the site is progressing well. Additional check-ins will be scheduled as needed during the osseointegration period before your final restoration is fabricated and placed.
